Equality Health Direct participates in the CMS ACO model, which connects organizations such as Equality Health Direct with healthcare professionals to provide improved care for patients throughout their health and wellness journey.

Equality Health Direct began participation in the CMS Innovation Center models on January 1, 2022 under the GPDC model. The ACO’s first Performance Year ended December 31, 2022.
